Dyslipidemia plays a crucial role in atherogenesis, in both prediabetes and diabetes. There persists a lacuna in the evaluation of postprandial lipid parameters in prediabetes.
To comparatively evaluate fasting and postprandial blood lipid parameters and atherogenic lipid ratios for cardiovascular risk assessment, in prediabetes and diabetes.
Fifty-one patients diagnosed with diabetes mellitus and thirty-two with prediabetes were selected for the study. Lipid profile and blood glucose were analyzed in fasting and postprandial blood samples.
Kolmogorov-Smirnov test, Shapiro-Wilk test, one-way ANOVA, and Pearson's regression analysis were applied.
Postprandially, triglycerides (TG) was increased significantly in diabetes compared to controls ( < 0.01) and prediabetics ( < 0.05). Among the lipid ratios, triglyceride/high density lipoprotein (TG/HDLc) was significantly increased postprandially in diabetes compared to controls ( < 0.05). A comparative analysis of fasting and postprandial parameters within each group showed a significant increase in postprandial TG/HDLc compared to the fasting state in prediabetes ( < 0.001) and diabetes ( < 0.001). Postprandial TG ( < 0.01) and TG/HDLc ( < 0.01) showed a stronger correlation with HbA compared to fasting TG ( < 0.05) and TG/HDLc ( > 0.05). The prevalence of dyslipidemia and insulin resistance was higher in postprandial state than the fasting state in prediabetes and diabetes.
Postprandial TG and the TG/HDLc reflect lipid abnormalities than the corresponding fasting variables in diabetes and prediabetes. Postprandial TG and TG/HDLc are better reflectors of cardiovascular status in prediabetes and diabetes.
血脂异常在糖尿病前期和糖尿病的动脉粥样硬化形成过程中起着关键作用。目前在糖尿病前期餐后血脂参数评估方面仍存在空白。
比较评估糖尿病前期和糖尿病患者的空腹及餐后血脂参数以及致动脉粥样硬化血脂比值,以进行心血管风险评估。
选取51例糖尿病患者和32例糖尿病前期患者进行研究。对空腹和餐后血样进行血脂谱和血糖分析。
应用柯尔莫哥洛夫-斯米尔诺夫检验、夏皮罗-威尔克检验、单因素方差分析和皮尔逊回归分析。
餐后,糖尿病患者的甘油三酯(TG)较对照组(P<0.01)和糖尿病前期患者(P<0.05)显著升高。在血脂比值中,糖尿病患者餐后甘油三酯/高密度脂蛋白(TG/HDLc)较对照组显著升高(P<0.05)。每组空腹和餐后参数的比较分析显示,糖尿病前期(P<0.001)和糖尿病(P<0.001)患者餐后TG/HDLc较空腹状态显著升高。与空腹TG(P<0.05)和TG/HDLc(P>0.05)相比,餐后TG(P<0.01)和TG/HDLc(P<0.01)与糖化血红蛋白(HbA)的相关性更强。糖尿病前期和糖尿病患者餐后血脂异常和胰岛素抵抗的患病率高于空腹状态。
与糖尿病和糖尿病前期相应的空腹变量相比,餐后TG和TG/HDLc更能反映血脂异常。餐后TG和TG/HDLc能更好地反映糖尿病前期和糖尿病患者的心血管状况。
