Condyloma acuminatum (CA) is both highly infectious and frequently recurring and requires long-term, repeated treatments, which seriously affect the physical and psychological health of patients. The purpose of the present study was to investigate the effectiveness of a combination therapy of traditional Chinese medicine and CO laser on CA and the relationship between CA relapse and cellular immunity.
The study cohort consisted of 160 CA patients who underwent ambulatory treatment between January 2017 and January 2019 in the Department of Dermatology and Venerology of our hospital. The 160 patients were randomly divided into two groups: a combination therapy group (80 cases), who underwent CO -laser treatment followed by three courses of oral traditional Chinese medicine and a control group (80 cases), who were only treated with the CO laser to remove warts. The efficacy of the CO -laser treatment was evaluated on the first month after treatment and relapse was evaluated at monthly follow-ups for 6 months. Additionally, 20 normal volunteers were also recruited. Three months before and after treatment, the cellular immunity factors of peripheral blood T lymphocyte subsets, including CD4+, CD8+, CD4+/CD8+, and interleukin-2 (IL-2), were detected and compared between CA patients and normal volunteers, the combination therapy and control groups, and the relapse and cure groups to determine whether there were statistical differences.
Compared with normal volunteers, CA patients exhibited lower CD4+, CD4+/CD8+, and IL-2 levels and higher CD8+ levels (P < .05). In addition, the rates of relapse for the combination therapy and control groups were 25.7% and 40.8%, respectively. However, the comprehensive immunity factors showed no statistical difference (P > .05) before treatment. Three months after treatment, factors including CD4+ and CD4+/CD8+ were higher in the combination therapy group than in the control group (P < .05), and CD8+ and IL-2 showed no statistical difference (P > .05); factors including CD4+, CD4+/CD8+, and IL-2 were higher and CD8+ was lower in the cure group than in the relapse group (P < .05).
The therapy combination of traditional Chinese medicine and CO -laser treatment can reduce the relapse rate of CA. It might be that traditional Chinese medicine combined with CO2-laser treatment regulate liver meridian, Qi and blood, and restore the balance between various subgroups.
尖锐湿疣(CA)具有高度传染性且常复发,需要长期、反复治疗,严重影响患者身心健康。本研究旨在探讨中药联合 CO 激光治疗 CA 的疗效及 CA 复发与细胞免疫的关系。
选取 2017 年 1 月至 2019 年 1 月我院皮肤科门诊接受门诊治疗的 160 例 CA 患者为研究对象,将其随机分为两组,联合治疗组(80 例)给予 CO 激光治疗后口服中药 3 个疗程,对照组(80 例)单纯给予 CO 激光去除疣体。治疗后 1 个月评价 CO 激光治疗效果,6 个月时进行每月随访评价复发情况。另选取 20 例健康志愿者作为正常对照组。治疗前后 3 个月,比较 CA 患者与正常对照组、联合治疗组与对照组、复发组与治愈组外周血 T 淋巴细胞亚群细胞免疫因子 CD4+、CD8+、CD4+/CD8+、白细胞介素-2(IL-2),判断其差异是否有统计学意义。
与正常对照组相比,CA 患者 CD4+、CD4+/CD8+、IL-2 水平较低,CD8+水平较高(P < .05)。联合治疗组与对照组复发率分别为 25.7%、40.8%,但治疗前综合免疫因子比较差异无统计学意义(P > .05)。治疗后 3 个月,联合治疗组 CD4+、CD4+/CD8+高于对照组(P < .05),CD8+、IL-2 差异无统计学意义(P > .05);治愈组 CD4+、CD4+/CD8+、IL-2 高于复发组,CD8+低于复发组(P < .05)。
中药联合 CO 激光治疗能降低 CA 的复发率,可能是通过中药联合 CO2 激光治疗调节肝经气血,恢复各亚群间的平衡。
