BACKGROUND
Isolated symptomatic cervical stenosis of the atlas is quite rare; there have been 11 cases reported in literature.
CASE DESCRIPTION
A 76-year-old male presented with myelopathy attributed to C1 arch stenosis. Neuroimaging studies revealed posterior atlas compression of the spinal cord. Following a cervical laminectomy involving excision of the arch of the atlas, and the patient's symptoms resolved.
CONCLUSION
C1 stenosis resulting in cervical myelopathy due to posterior compression from the arch of the atlas is easily missed. Notably, C1 arch laminectomy may be very effective in resolving this entity.

孤立性症状性寰椎颈椎管狭窄相当罕见；文献中已报道11例。
病例描述
一名76岁男性因C1椎弓狭窄出现脊髓病。神经影像学研究显示脊髓受到寰椎后方压迫。在进行包括切除寰椎椎弓的颈椎椎板切除术后，患者症状缓解。
结论
由于寰椎椎弓后方压迫导致的C1狭窄引起的颈椎脊髓病很容易被漏诊。值得注意的是，寰椎椎弓椎板切除术可能对解决这一问题非常有效。
