Esophageal adenocarcinoma (EAC) is one of the main subtypes of esophageal cancer. The incidence rate of EAC increased progressively while the 5-year relative survival rates were poor in the past two decades. The mechanism of EAC has been studied extensively in relation to genetic factors, but less so with respect to human microbiota. Currently, researches about the relationship between EAC and the human microbiota is a newly emerging field of study. Herein, we present the current state of knowledge linking human microbiota to esophageal adenocarcinoma and its precursor lesion-gastroesophageal reflux disease and Barrett's esophagus. There are specific human bacterial alternations in the process of esophageal carcinogenesis. And bacterial dysbiosis plays an important role in the process of esophageal carcinogenesis inflammation, microbial metabolism and genotoxicity. Based on the human microbiota alternation in the EAC cascade, it provides potential microbiome-based clinical application. This review is focused on novel targets in prevention, diagnosis, prognosis, and therapy for esophageal adenocarcinoma.
食管腺癌(EAC)是食管癌的主要亚型之一。在过去二十年中,EAC的发病率逐渐上升,而5年相对生存率却很低。关于EAC的机制,已经针对遗传因素进行了广泛研究,但对于人类微生物群的研究较少。目前,关于EAC与人类微生物群之间关系的研究是一个新兴领域。在此,我们阐述了将人类微生物群与食管腺癌及其前驱病变——胃食管反流病和巴雷特食管相联系的当前知识状态。在食管癌发生过程中存在特定的人类细菌变化。并且细菌生态失调在食管癌发生过程中的炎症、微生物代谢和基因毒性方面发挥着重要作用。基于EAC级联反应中的人类微生物群变化,它提供了基于微生物群的潜在临床应用。本综述聚焦于食管腺癌预防、诊断、预后和治疗的新靶点。
