OBJECTIVE
To systematically evaluate the impact of diabetes on the prognosis of bladder cancer patients after radical cystectomy (RC).
METHODS
PubMed, Embase, and Cochrane Library databases were selected from inception to October 2021. The studies on the effects of diabetes on bladder cancer patients after RC were included for analysis. The inclusion and exclusion criteria were independently selected for literature screening, the quality of the included studies was evaluated, and data were extracted.
RESULTS
A total of 5 cohort studies were included, with a total of 2 661 subjects, including 391 cases in the diabetic group, non-diabetes. Meta-analysis results show that diabetes increases the overall risk of death in patients after RC (HR = 1.36, 95% CI: 1.30 ∼ 1.43, < 0.001) and the risk of tumor-specific death (HR = 1.59, 95% CI: 1.29 ∼ 1.95, < 0.001). Sensitivity analysis shows that the stability of this study is well.
CONCLUSION
Diabetes was an independent risk factor in terms of overall and cancer-specific survival in patients who underwent RC.
目的
系统评价糖尿病对根治性膀胱切除术（RC）后膀胱癌患者预后的影响。
方法
选取PubMed、Embase和Cochrane图书馆数据库自建库至2021年10月的数据。纳入分析糖尿病对RC术后膀胱癌患者影响的研究。独立选择纳入和排除标准进行文献筛选，评估纳入研究的质量并提取数据。
结果
共纳入5项队列研究，共2661名受试者，其中糖尿病组391例，非糖尿病组……荟萃分析结果显示，糖尿病增加了RC术后患者的总体死亡风险（HR = 1.36，95%CI：1.30～1.43，<0.001）和肿瘤特异性死亡风险（HR = 1.59，95%CI：1.29～1.95，<0.001）。敏感性分析表明本研究稳定性良好。
结论
就接受RC治疗的患者的总体生存和癌症特异性生存而言，糖尿病是一个独立的危险因素。
