Abstract

OBJECTIVE

The aim of this study was to explore the clinical application value of two-dimensional ultrasound (2D-US) combined with contrast-enhanced ultrasonography (CEUS) in the differential diagnosis of benign, borderline ovarian tumors (BOTs), and malignant ovarian epithelial tumors (OETs).

METHODS

The clinical data of 108 patients who underwent surgery for pathologically confirmed of OETs at Peking University People's Hospital between December 2018 and November 2023 were retrospectively studied. The diagnostic value of 2D-US combined with CEUS for diagnosing OETs was analyzed using chi-square tests, receiver operating characteristic (ROC) curves, and random forest models.

RESULTS

Among the 108 cases of OETs, 23 were benign, 34 were BOTs, and 51 were malignant. Chi-square tests confirmed that the perfusion pattern of the contrast agent plays an important role in the differential diagnosis of OETs. Compared with those in the benign group, the BOTs were not significantly different in terms of perfusion phase and enhancement intensity, but the regression time of the BOTs was earlier (P < 0.05). Compared with the BOTs, the malignant tumors group showed earlier perfusion and higher enhancement intensity, with no significant difference in regression time. The ROC curve results indicated that the combined diagnostic efficiency of 2D-US and CEUS in distinguishing OETs was significantly higher than that of a single diagnostic technique in terms of sensitivity, specificity, accuracy, and AUC. The random forest model results revealed that among the various parameters used in the differential diagnosis of OETs, the perfusion pattern was the most significant factor.

CONCLUSION

2D-US combined with CEUS helps improve the differential diagnostic efficiency for benign, BOTs, and malignant OETs.

目的

本研究旨在探讨二维超声(2D-US)联合超声造影(CEUS)在鉴别卵巢良、交界性肿瘤(BOTs)和恶性上皮性卵巢肿瘤(OETs)中的临床应用价值。

方法

回顾性分析 2018 年 12 月至 2023 年 11 月北京大学人民医院经手术病理证实为 OETs 的 108 例患者的临床资料。采用卡方检验、受试者工作特征(ROC)曲线和随机森林模型分析 2D-US 联合 CEUS 对 OETs 诊断的价值。

结果

108 例 OETs 中,良性 23 例,BOTs34 例,恶性 51 例。卡方检验证实,造影剂灌注模式在 OETs 鉴别诊断中起重要作用。与良性组相比,BOTs 组在灌注期和增强强度方面无显著差异,但 BOTs 组的廓清时间更早(P<0.05)。与 BOTs 组相比,恶性肿瘤组的灌注更早,增强强度更高,廓清时间无明显差异。ROC 曲线结果表明,2D-US 联合 CEUS 鉴别 OETs 的诊断效能在敏感性、特异性、准确性和 AUC 方面明显高于单一诊断技术。随机森林模型结果显示,在鉴别 OETs 的各项参数中,灌注模式是最重要的因素。

结论

2D-US 联合 CEUS 有助于提高对良、BOTs 和恶性 OETs 的鉴别诊断效率。
