Abstract

OBJECTIVE

To observe the effect of point-toward-point insertion of elongated needle on post-stroke fatigue and explore its underlying mechanism.

METHODS

Sixty-four patients with post-stroke fatigue were randomly divided into an observation group (32 cases, 2 cases dropped out) and a control group (32 cases, 2 cases dropped out). In addition to the conventional treatment of western medicine and rehabilitation exercises, (regulating the governor vessel and the spirit) therapy of acupuncture was used in the control group, and acupuncture was applied to Baihui (GV 20), Sishencong (EX-HN 1), Shenting (GV 24), Fengfu (GV 16) and bilateral Fengchi (GB 20) and the needles were retained for 30 min in one treatment. In the observation group, besides the interventions as the control group, point-toward-point insertion of elongated needle was adopted. The needle was inserted from Zhiyang (GV 9) toward Dazhui (GV 14), from Shendao (GV 11) toward Yaoyangguan (GV 3) and from Yaoqi (EX-B 9) toward Yaoyangguan (GV 3); and the needles were retained for 30 min in one treatment. In the two groups, the acupuncture was delivered once daily, 6 times a week, consecutively for 4 weeks. The scores of the fatigue severity scale (FSS), Pittsburgh sleep quality index (PSQI), Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD), and Fugl-Meyer assessment (FMA) were observed before and after treatment completion in the two groups. Using ELISA, the levels of serum interleukin (IL)-1β, IL-6, IL-10, high-sensitivity C-reactive protein (hs-CRP), and homocysteine (Hcy) were detected before and after treatment completion; and clinical effect was evaluated.

RESULTS

The scores of FSS, PSQI, HAMA and HAMD, as well as the levels of serum IL-1β, IL-6, hs-CRP and Hcy were reduced (<0.05), and the scores of FMA and the levels of serum IL-10 were increased (<0.05) after treatment in comparison with those before treatment in the two groups. Compared with the control group, in the observation group, the scores of FSS, PSQI, HAMA and HAMD, and the levels of serum IL-1β, IL-6, hs-CRP and Hcy were reduced (<0.05, <0.01), and the score of FMA and the level of serum IL-10 were increased (<0.05). The total clinical effective rate was 83.3% (25/30) in the observation group, which was higher than 73.3% (22/30) of the control group (<0.05).

CONCLUSION

The point-toward-point insertion of elongated needle alleviates fatigue, anxiety and depression, and ameliorates sleep and motor function in the patients with post-stroke fatigue. It may be related to the attenuation of inflammatory responses.

目的

观察芒针透刺法对脑卒中后疲劳的影响并探讨其潜在机制。

方法

将64例脑卒中后疲劳患者随机分为观察组(32例,脱落2例)和对照组(32例,脱落2例)。对照组在西医常规治疗及康复训练基础上采用针刺(调督醒脑)疗法,针刺百会(GV 20)、四神聪(EX-HN 1)、神庭(GV 24)、风府(GV 16)及双侧风池(GB 20),留针30分钟,每日1次,每周6次,连续治疗4周。观察组在对照组干预基础上采用芒针透刺法,分别从至阳(GV 9)透刺大椎(GV 14)、神道(GV 11)透刺腰阳关(GV 3)、腰奇(EX-B 9)透刺腰阳关(GV 3),留针30分钟,每日1次,每周6次,连续治疗4周。观察两组治疗前后疲劳严重程度量表(FSS)、匹兹堡睡眠质量指数(PSQI)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)及Fugl-Meyer评估量表(FMA)评分,采用酶联免疫吸附测定法(ELISA)检测两组治疗前后血清白细胞介素(IL)-1β、IL-6、IL-10、高敏C反应蛋白(hs-CRP)及同型半胱氨酸(Hcy)水平,并评价临床疗效。

结果

两组治疗后FSS、PSQI、HAMA及HAMD评分,血清IL-1β、IL-6、hs-CRP及Hcy水平均降低(<0.05),FMA评分及血清IL-10水平升高(<0.05)。与对照组比较,观察组治疗后FSS、PSQI、HAMA及HAMD评分,血清IL-1β、IL-6、hs-CRP及Hcy水平降低更明显(<0.05,<0.01),FMA评分及血清IL-10水平升高更明显(<0.05)。观察组总有效率为83.3%(25/30),高于对照组的73.3%(22/30)(<0.05)。

结论

芒针透刺法可缓解脑卒中后疲劳患者的疲劳、焦虑及抑郁症状,改善睡眠及运动功能,其机制可能与减轻炎症反应有关。
