Abstract

OBJECTIVES

This study aimed to determine the prevalence and genotyping of human papillomavirus (HPV) and to assess co-infection with Epstein-Barr virus (EBV) in oral cavity and oropharyngeal cancers (OC and OPC) specimens from patients at a tertiary care hospital in Northeastern Mexico.

METHODS

Formalin-fixed and paraffin-embedded tumor specimens from 41 patients with OC and OPC were evaluated. HPV detection and genotyping were performed using the Ampliquality HPV-Type Express kit. EBV DNA detection was carried out by real-time quantitative polymerase chain reaction.

RESULTS

HPV DNA was detected in 14 (34.1%) specimens, with a higher prevalence in OC (78.6%) compared with OPC (21.4%). HPV-16 was the most frequently identified genotype (92.9%), found as a single infection in 53.8% of cases and co-infection with other genotypes in 46.2%. EBV DNA was detected in six (14.6%) specimens, with OC being the most common site. Co-infection with HPV and EBV was observed in only one case. Statistical significance was found between HPV infection and smoking history ( = 0.020) and between EBV infection and patient age ( = 0.026).

CONCLUSIONS

Our results reveal a higher prevalence of HPV infection in OC compared with OPC, with HPV-16 being the predominant genotype. HPV-positive cases were predominantly found in older male patients. Thus, expanding HPV vaccination to broader populations could potentially impact cancer incidence. EBV co-infection with HPV was infrequent, and further research is needed to fully understand the role of these viruses in OC and OPC development.

目的

本研究旨在确定墨西哥东北部一家三级护理医院患者口腔癌和口咽癌(OC和OPC)标本中人类乳头瘤病毒(HPV)的流行率和基因分型,并评估与爱泼斯坦-巴尔病毒(EBV)的合并感染情况。

方法

对41例OC和OPC患者的福尔马林固定石蜡包埋肿瘤标本进行评估。使用Ampliquality HPV-Type Express试剂盒进行HPV检测和基因分型。通过实时定量聚合酶链反应进行EBV DNA检测。

结果

在14份(34.1%)标本中检测到HPV DNA,OC中的流行率(78.6%)高于OPC(21.4%)。HPV-16是最常鉴定出的基因型(92.9%),在53.8%的病例中为单一感染,在46.2%的病例中与其他基因型合并感染。在6份(14.6%)标本中检测到EBV DNA,OC是最常见的部位。仅在1例病例中观察到HPV和EBV合并感染。在HPV感染与吸烟史之间(P = 0.020)以及EBV感染与患者年龄之间(P = 0.026)发现有统计学意义。

结论

我们的结果显示,与OPC相比,OC中HPV感染的流行率更高,HPV-16是主要基因型。HPV阳性病例主要见于老年男性患者。因此,将HPV疫苗接种扩大到更广泛人群可能会对癌症发病率产生潜在影响。HPV与EBV的合并感染并不常见,需要进一步研究以充分了解这些病毒在OC和OPC发生发展中的作用。
