Abstract

OBJECTIVE

A cross-sectional survey was conducted to investigate the differences in preferences, gateway effects, and potential motivations for e-cigarette use among different genders of e-cigarette users in China, thereby providing ideas for the development of public prevention and intervention measures for e-cigarettes.

METHODS

This study adopted a combination of online web questionnaires and offline distribution questionnaires to survey 640 e-cigarette users by snowball sampling and convenience sampling in China in 2023. We used the Potential E-Cigarette Dependence Scale, the E-Cigarette Preference Scale, and the General Information Scale to conduct the survey and assessment, and surveyed 360 individuals of those who only used traditional cigarettes for comparison.

RESULTS

(1) The total number of participants in this research study was 1792, the mean age was 26.9 ± 9.0 years and the number of e-cigarette users was 640, of which 535 (83.6%) were males and 105 (16.4%) were females. (2) There was a statistically significant association between e-cigarette flavor preferences (flavor/nicotine) and gender ( < 0.05). Compared to males, females preferred e-cigarettes with scented/tobacco/fruit/beverage/nicotine (0 mg/12 mg) flavors. (3) Compared with males, females were more likely to "Transition to using traditional cigarettes after smoking e-cigarettes (Gateway effect)" ( < 0.05). (4) Whereas males were more likely to experience "Craving for traditional cigarettes after using e-cigarettes (Induction effect)" ( < 0.05). (5) There were significant gender differences in motivations for e-cigarette use. Males were more likely to use e-cigarettes to "quit traditional cigarettes," whereas e-cigarette use was more closely related to self-perceptions of "feeling cool." (6) Multi-factorial logistic regression analysis showed that there was a significant correlation between "e-cigarette liquid flavor (tobacco/fruit/beverage), e-cigarette liquid nicotine concentration (0 mg/12 mg)" and "Gender" ( < 0.05); Gateway effect was significantly correlated with "Gender and Age"( < 0.05); E-cigarette addiction dependence was not significantly correlated with gender; Mild addiction to e-cigarettes was significantly correlated with "hope e-cigarettes carry nicotine, and the motivations for choosing e-cigarettes (to quit traditional cigarettes)" ( < 0.05). Severe addiction to e-cigarettes was significantly correlated with "hope e-cigarettes carry nicotine, and the motivations for choosing e-cigarettes (unable to use traditional cigarettes in public)" ( < 0.05).

CONCLUSION

Among Chinese e-cigarette users, females preferred e-cigarettes with special flavors, and either without or with middling concentrations of nicotine. The gateway effect was more prominent in females and adolescents, and the induction effect was more notable in males. There was no significant correlation between addiction dependence on e-cigarettes and gender. E-cigarette use was more likely to be motivated by a desire to quit using traditional cigarettes in males, whereas women were more likely to be motivated by "self-perception."

开展一项横断面调查,以探究中国不同性别的电子烟使用者在偏好、入门效应及使用电子烟的潜在动机方面的差异,从而为制定电子烟公共预防和干预措施提供思路。

方法

本研究采用网络问卷和线下发放问卷相结合的方式,于2023年在中国通过雪球抽样和便利抽样对640名电子烟使用者进行调查。我们使用潜在电子烟依赖量表、电子烟偏好量表和一般信息量表进行调查和评估,并对360名仅使用传统香烟的个体进行调查以作比较。

结果

(1)本研究的参与者总数为1792人,平均年龄为26.9±9.0岁,电子烟使用者为640人,其中535人(83.6%)为男性,105人(16.4%)为女性。(2)电子烟口味偏好(口味/尼古丁)与性别之间存在统计学显著关联(<0.05)。与男性相比,女性更喜欢带有香味/烟草/水果/饮料/尼古丁(0毫克/12毫克)口味的电子烟。(3)与男性相比,女性更有可能“在吸电子烟后转而使用传统香烟(入门效应)”(<0.05)。(4)而男性更有可能经历“吸电子烟后渴望吸传统香烟(诱导效应)”(<0.05)。(5)电子烟使用动机存在显著性别差异。男性更有可能出于“戒掉传统香烟”的目的使用电子烟,而女性使用电子烟则与“感觉很酷”的自我认知关系更为密切。(6)多因素逻辑回归分析表明,“电子烟液口味(烟草/水果/饮料)、电子烟液尼古丁浓度(0毫克/12毫克)”与“性别”之间存在显著相关性(<0.05);入门效应与“性别和年龄”显著相关(<0.05);电子烟成瘾依赖性与性别无显著相关性;轻度电子烟成瘾与“希望电子烟含有尼古丁以及选择电子烟的动机(戒掉传统香烟)”显著相关(<0.05)。重度电子烟成瘾与“希望电子烟含有尼古丁以及选择电子烟的动机(在公共场合无法使用传统香烟)”显著相关(<0.05)。

结论

在中国电子烟使用者中,女性更喜欢带有特殊口味、不含或含有中等浓度尼古丁的电子烟。入门效应在女性和青少年中更为突出,诱导效应在男性中更为明显。电子烟成瘾依赖性与性别无显著相关性。男性使用电子烟更可能是出于戒掉传统香烟的愿望,而女性则更可能是受“自我认知”的驱动。
