Acute respiratory component of tardive dyskinesia, though reported very infrequently, is a potentially lethal condition necessitating urgent intervention. Several relevant issues are discussed with an illustrative case material. The need to differentiate respiratory dyskinesia from psychogenic hyperventilation is emphasized. Use of neuroleptics is discouraged to avoid unfavourable long-term outcome.
