Rupture of the patellar tendon is an uncommon injury that severely compromises knee function, with re-rupture rates varying between 2% and 50% depending on the repair technique. Currently, there is no consensus on the optimal method for revision patellar tendon repair. This Technical Note presents an alternative approach using allograft Achilles tendon augmentation after an initial procedure involving transosseous tunnels with hamstring tendons in the tibia and patella. By using a homologous Achilles tendon graft, this method enhances stability and reduces the risk of re-rupture. Detailed surgical steps and a postoperative rehabilitation protocol are included to optimize patient outcomes, making this a valuable addition to existing surgical approaches for patellar tendon reconstruction.
