Reconstruction of the nasal ala poses challenges due to the need to maintain its concave 3-dimensional shape, preserve nasal symmetry with the contralateral ala, and retain functional patency of the nasal valve. Full-thickness defects after Mohs surgery that involve the external skin, fibrofatty middle portion, and internal nasal lining often require staged procedures for an acceptable cosmetic outcome. Herein, we describe a single-stage repair of the nasal ala using a subcutaneous pedicle advancement flap with a 180-degree transposition.
