A new case of refractory anemia with excess of blasts (RAEB) with acquired Hemoglobin H disease (HbH) evolving into acute nonlymphoid leukemia (ANLL) is reported. In this patient, both family and personal history were negative for alpha thalassemia. At time of diagnosis of RAEB, 'golf ball' erythrocytic inclusions, typical of HbH, were found. The cellulose acetate electrophoresis (pH 8.9) revealed a fast moving band, corresponding to HbH. An evolution into ANLL, M1 subtype, 25 months later, has been documented.
报告了1例难治性贫血伴原始细胞增多（RAEB）合并获得性血红蛋白H病（HbH）并演变为急性非淋巴细胞白血病（ANLL）的新病例。该患者的家族史和个人史中α地中海贫血均为阴性。在诊断RAEB时，发现了典型的HbH“高尔夫球”样红细胞包涵体。醋酸纤维素电泳（pH 8.9）显示有一条快速移动带，对应于HbH。记录显示25个月后该病例演变为ANLL的M1亚型。
