Abstract

OBJECTIVE

Determine the serum levels of endogenous secretory receptor for advanced glycation end products (esRAGEs) in patients with type 2 diabetes and mild cognitive impairment (MCI) and in control patients with type 2 diabetes but no MCI, and examine the relationship of esRAGE and MCI with other clinical factors.

RESEARCH DESIGN AND METHODS

A total of 101 patients with type 2 diabetes who were hospitalized in the Department of Endocrinology at Fujian Provincial Hospital between January 2010 and January 2011 were enrolled. There were 58 patients with MCI and 43 patients without MCI (control). Serum levels of esRAGE were measured using an enzyme-linked immunosorbent assay (ELISA). Other clinical parameters were also measured.

RESULTS

Type 2 diabetic patients with MCI had a longer duration of diabetes; elevated HbA(1c), total cholesterol (CHOL), LDL cholesterol (LDL-C), triglyceride (TG), intima-media thickness, C-reactive protein (CRP), and brachial-ankle pulse wave velocity (ba-PWV); and lower ankle brachial index (ABI) and esRAGE relative to the control group. Among patients with MCI, the Montreal Cognitive Assessment (MoCA) score was positively correlated with serum esRAGE but negatively correlated with CHOL. Spearman rank correlation analysis indicated that esRAGE was positively correlated with MoCA score and ABI but negatively correlated with ba-PWV, CHOL, TG, and CRP in all subjects.

CONCLUSIONS

Our results suggest that esRAGE may be a potential protective factor for dyslipidemia, atherosclerosis, and MCI in patients with type 2 diabetes.

目的

测定 2 型糖尿病伴轻度认知障碍(MCI)患者及单纯 2 型糖尿病无 MCI 患者的血清内源性晚期糖基化终产物受体(esRAGE)水平,探讨 esRAGE 与 MCI 及其它临床因素的关系。

研究设计和方法

选取 2010 年 1 月至 2011 年 1 月在福建省立医院内分泌科住院的 101 例 2 型糖尿病患者,其中 MCI 患者 58 例,无 MCI 患者 43 例。采用酶联免疫吸附法(ELISA)检测血清 esRAGE 水平,同时检测其它临床参数。

结果

与对照组相比,伴 MCI 的 2 型糖尿病患者糖尿病病程较长,HbA1c、总胆固醇(CHOL)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、颈动脉内膜中层厚度、C 反应蛋白(CRP)和踝臂脉搏波速度(ba-PWV)升高,而踝肱指数(ABI)和 esRAGE 降低。伴 MCI 的患者中,蒙特利尔认知评估(MoCA)评分与血清 esRAGE 呈正相关,与 CHOL 呈负相关。Spearman 秩相关分析显示,在所有患者中,esRAGE 与 MoCA 评分和 ABI 呈正相关,与 ba-PWV、CHOL、TG 和 CRP 呈负相关。

结论

本研究结果提示,esRAGE 可能是 2 型糖尿病患者发生血脂异常、动脉粥样硬化和 MCI 的潜在保护因素。
