Asymptomatic lesions of the pancreas, referred to as 'incidentalomas', have appeared with increased frequency in recent years. Giant incidentalomas have rarely been reported in the literature.
We report herein a rare case of a giant cystic pancreatic incidentaloma measuring 12.7cm×8cm, which was found in an otherwise healthy male patient during a routine genitourinary imaging work-up. The patient underwent a distal pancreatectomy and splenectomy; the pathology report demonstrated a giant serous cystadenoma of the body and tail of the pancreas.
The management of pancreatic incidentalomas is challenging. While solid lesions almost always warrant surgery, there is ongoing debate concerning the management of cystic lesions that are found incidentally in the pancreas and have no clinical manifestations.
We report herein an interesting case of a voluminous incidental cystic pancreatic lesion. The appropriate approach and the decision whether to operate or not in such cases can be puzzling to the physician.
胰腺无症状性病变,即“偶发瘤”,近年来出现的频率有所增加。巨大偶发瘤在文献中鲜有报道。
我们在此报告一例罕见的巨大囊性胰腺偶发瘤病例,瘤体大小为12.7cm×8cm,在一名身体健康的男性患者进行常规泌尿生殖系统影像学检查时被发现。患者接受了胰腺远端切除术和脾切除术;病理报告显示为胰腺体尾部巨大浆液性囊腺瘤。
胰腺偶发瘤的处理具有挑战性。虽然实性病变几乎总是需要手术治疗,但对于胰腺偶然发现且无临床表现的囊性病变的处理,目前仍存在争议。
我们在此报告一例有趣的大量胰腺偶发囊性病变病例。在这种情况下,合适的处理方法以及是否进行手术的决策可能会让医生感到困惑。
