Abstract

Antifungal resistance and virulence properties of are a growing health problem worldwide. To study the expression of virulence and azole resistance genes in 39 clinical strains of , we used a model of infection of human vaginal epithelial cells with strains isolated from Mexican women with vulvovaginal candidiasis (VVC). The strains were identified by PCR amplification of the ITS1 and ITS2 regions of rRNA. The detection and expression of virulence genes and azole resistance genes and were performed using PCR and RT-PCR, respectively. All strains were sensitive to nystatin and 38 (97.4%) and 37 (94.9%) were resistant to ketoconazole and fluconazole, respectively. , , , , , , , , , and - were present in all strains; was identified in 37 (94.8%) isolates, in 35 (89.7%), in 14 (35.8%), and in 26 (66.6%). In nearly all of the strains, , , , , , and were expressed, whereas was expressed in 20 (51.3%) and in 14 (35.8%). In our in vitro model of infection with , the clinical strains showed different expression profiles of virulence genes in association with the azole resistance gene . The results indicate that the strains that infect Mexican patients suffering from VVC are highly virulent and virtually all are insensitive to azoles.

念珠菌的抗真菌耐药性和毒力特性在全球范围内日益成为一个健康问题。为了研究39株临床念珠菌中毒力和唑类耐药基因的表达情况,我们使用了从患有外阴阴道念珠菌病(VVC)的墨西哥女性中分离出的念珠菌菌株感染人阴道上皮细胞的模型。通过rRNA的ITS1和ITS2区域的PCR扩增来鉴定菌株。分别使用PCR和RT-PCR检测毒力基因和唑类耐药基因ERG11和CDR1的表达。所有菌株对制霉菌素敏感,分别有38株(97.4%)和37株(94.9%)对酮康唑和氟康唑耐药。ALS1、ALS3、SAP1、SAP2、SAP3、SAP4、SAP5、SAP6、SAP7、SAP8和PLB1在所有菌株中均存在;37株(94.8%)分离株中鉴定出MP65,35株(89.7%)中鉴定出HWP1,14株(35.8%)中鉴定出ECE1,26株(66.6%)中鉴定出PRA1。在几乎所有菌株中,ALS1、ALS3、SAP1、SAP2、SAP3、SAP4均有表达,而MP65在20株(51.3%)中表达,ECE1在14株(35.8%)中表达。在我们的体外念珠菌感染模型中,临床菌株显示出与唑类耐药基因ERG11相关的毒力基因不同表达谱。结果表明,感染患有VVC的墨西哥患者的菌株具有高毒力,并且几乎所有菌株对唑类均不敏感。
