Abstract

PURPOSE

To compare the abilities of turbo spin-echo diffusion-weighted imaging (TSE DWI) and multi-shot echo planar DWI (MSh DWI) to discriminate orbital lymphoma from inflammatory lesions.

MATERIALS AND METHODS

Twenty-nine patients with pathologically confirmed lymphomas and 39 patients with inflammation were imaged with a 3.0-T system. The apparent diffusion coefficient (ADC) of each lesion was measured. Signal intensities compared to normal grey matter on conventional imaging were also measured.

RESULTS

The ADCs derived from the TSE DWI of the lymphomas (0.68 ± 0.14 × 10 mm/s) were significantly lower than those of the inflammation cases (1.04 ± 0.39 × 10 mm/s; p < 0.001). The ADCs derived from MSh DWI could not be used to separate the lymphomas from the inflammation (1.16 ± 0.43 × 10 mm/s vs. 1.36 ± 0.48 × 10 mm/s; p = 0.06). Conventional sequences also could not separate the lymphomas from the inflammation (p > 0.05). The ROC analysis showed the best diagnostic performance with ADCs derived from TSE DWI (the area under the curve: AUC = 0.831) followed by ADC derived from MSh DWI (AUC = 0.633).

CONCLUSION

The ADCs derived from TSE DWI might help to differentiate orbital lymphomas from inflammation.

目的

比较涡轮自旋回波扩散加权成像(TSE DWI)和多回波平面扩散加权成像(MSh DWI)区分眼眶淋巴瘤与炎症性病变的能力。

材料与方法

对 29 例经病理证实的淋巴瘤患者和 39 例炎症患者进行 3.0T 系统成像。测量每个病变的表观扩散系数(ADC)。还测量了与常规成像中正常灰质相比的信号强度。

结果

TSE DWI 得出的淋巴瘤 ADC(0.68±0.14×10mm/s)显著低于炎症组(1.04±0.39×10mm/s;p<0.001)。MSh DWI 得出的 ADC 不能将淋巴瘤与炎症区分开来(1.16±0.43×10mm/s 与 1.36±0.48×10mm/s;p=0.06)。常规序列也无法将淋巴瘤与炎症区分开来(p>0.05)。ROC 分析显示,TSE DWI 得出的 ADC(曲线下面积:AUC=0.831)具有最佳的诊断性能,其次是 MSh DWI 得出的 ADC(AUC=0.633)。
