Abstract
Tension pneumomediastinum is a rare and life-threatening complication of mediastinal emphysema which can occur with mechanical ventilation. We present a case of tension mediastinum associated with mechanical ventilation in a patient with Acute Respiratory Distress Syndrome due to pneumonia. We discuss the mechanism and pathophysiology of tension pneumomediastinum, the potential association with pneumonia and recruitment manouvres, and its definitive emergency treatment.

张力性纵隔气肿是纵隔气肿的一种罕见且危及生命的并发症，可发生于机械通气时。我们报告一例因肺炎导致急性呼吸窘迫综合征的患者发生与机械通气相关的张力性纵隔气肿的病例。我们讨论了张力性纵隔气肿的机制和病理生理学、与肺炎及肺复张手法的潜在关联以及其明确的紧急治疗方法。
