Abstract

OBJECTIVE

To define the role of the EEG in predicting recurrence of febrile seizures (FS) in children after a first FS.

METHODS

Children with a first simple or complex FS who underwent EEG at our hospital were retrospectively enrolled. EEG recordings were classified in three groups: normal, abnormal (slow activity or epileptiform discharges), and pseudo-petit mal discharge (PPMD) pattern. Children were followed-up for at least three years.

RESULTS

A total of 126 patients met the entry criteria, and 113 of them completed the follow-up. Risk of FS recurrence decreased linearly with increasing age (-2% per month). The risk was higher among patients with PPMD pattern (absolute risk 86%, adjusted relative risk 2.00) and abnormal EEG (epileptiform discharges: absolute risk 71%, adjusted relative risk 2.00; slow activity: absolute risk 56%, adjusted relative risk 1.44), compared with those with normal EEG (absolute risk 41%).

CONCLUSIONS

PPMD and abnormal EEG should be considered as an independent risk factor for FS recurrence.

目的

明确脑电图(EEG)在预测儿童首次热性惊厥(FS)后复发中的作用。

方法

回顾性纳入在我院接受EEG检查的首次发生单纯性或复杂性FS的儿童。EEG记录分为三组:正常、异常(慢波活动或癫痫样放电)和假小发作放电(PPMD)模式。对儿童进行至少三年的随访。

结果

共有126例患者符合纳入标准,其中113例完成随访。FS复发风险随年龄增长呈线性下降(每月-2%)。与EEG正常的患者(绝对风险41%)相比,PPMD模式患者(绝对风险86%,校正相对风险2.00)和EEG异常(癫痫样放电:绝对风险71%,校正相对风险2.00;慢波活动:绝对风险56%,校正相对风险1.44)患者的复发风险更高。

结论

PPMD和EEG异常应被视为FS复发的独立危险因素。
