Cognitive impairment is common in Parkinson's disease (PD) and PD patients with mild cognitive impairment (PD-MCI) are at increased risk of developing Parkinson's disease dementia (PDD). Reliable biomarkers are required for objective identification of cognitive decline in PD. In this pilot study, serum levels of well-known mediators of neuroinflammation were measured in PD patients with or without MCI to find out the involvement of neuroinflammation and microglial activation in PD-MCI.
36 PD-MCI, 25 PD patients with normal cognition (PD-NC) and 19 healthy controls were recruited. Serum levels of NLR family pyrin domain containing 1 (NLRP1), NLRP3, caspase-1, NF-kB, IL-1b and IL-18 were measured by ELISA and a panel of neuropsychological tests was administered.
PD-MCI patients showed significantly reduced levels of NF-kB, IL-1b and IL-18, whereas NLRP1, NLRP3 and caspase-1 levels were comparable among PD-NC and PD-MCI patients. IL-18 levels were positively correlated with Addenbrooke's Cognitive Examination-Revised and Symbol Digit Modalities Test scores.
Levels of several microglial activation mediators are reduced in PD-MCI patients inferring a protective role to certain inflammation factors against cognitive decline in PD.
认知障碍在帕金森病(PD)中很常见,轻度认知障碍的帕金森病患者(PD-MCI)患帕金森病痴呆(PDD)的风险增加。需要可靠的生物标志物来客观识别帕金森病患者的认知衰退。在这项初步研究中,对有或无MCI的帕金森病患者的血清中著名的神经炎症介质水平进行了测量,以了解神经炎症和小胶质细胞激活在PD-MCI中的作用。
招募了36例PD-MCI患者、25例认知正常的帕金森病患者(PD-NC)和19名健康对照者。通过酶联免疫吸附测定法(ELISA)测量血清中含NLR家族吡咯结构域蛋白1(NLRP1)、NLRP3、半胱天冬酶-1、核因子-κB(NF-κB)、白细胞介素-1β(IL-1β)和白细胞介素-18的水平,并进行一组神经心理学测试。
PD-MCI患者的NF-κB、IL-1β和IL-18水平显著降低,而NLRP1、NLRP3和半胱天冬酶-1水平在PD-NC和PD-MCI患者中相当。IL-18水平与修订版的Addenbrooke认知检查及符号数字模式测验得分呈正相关。
PD-MCI患者中几种小胶质细胞激活介质的水平降低,提示某些炎症因子对帕金森病认知衰退具有保护作用。
