The study is aimed at studying the association between the levels of serum adiponectin (ADPN), high-sensitivity C-reactive protein (hs-CRP), and soluble intercellular adhesion molecule-1 (sICAM-1) and hypertensive cerebrovascular complications. 50 patients with hypertensive cerebrovascular disease treated in Gansu Provincial Hospital from December 2016 to December 2018 were selected as the experimental group, and 50 normal people who underwent physical examination were selected as the control group. The blood pressure, heart rate, and the complications were recorded, and the serum blood lipid indexes were detected. Moreover, the content of serum ADPN, hs-CRP, and sICAM-1; the neurological indexes; brain-derived neurotrophic factor (BNDF); and neurone-specific enolase (NSE) were also determined using ELISA. The content of aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen (BUN), and serum creatinine (SCR) in the experimental group was significantly higher than that in control group ( < 0.05); the incidence of cerebrovascular complications, systolic blood pressure, diastolic blood pressure, and heart rate increased ( < 0.05); the content of total cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL), high-density lipoprotein (HDL), hs-CRP, and sICAM-1 obviously rose ( < 0.05); and the content of ADPN and HDL obviously declined ( < 0.05). Besides, the experimental group had evidently lower systolic blood flow velocity (Vs), diastolic blood flow velocity (Vd), and mean blood flow velocity (Vm) and evidently higher pulsatility index (PI) ( < 0.05). The levels of S100 and NSE in the experimental group increased significantly, and the level of BNDF decreased significantly ( < 0.05). In patients with hypertensive cerebrovascular disease, the level of ADPN declines; the levels of hs-CRP and sICAM-1 rise; the incidence rate of cerebrovascular complications is elevated; and there are changes in the blood lipid, cerebrovascular hemodynamic, and neurological indexes, thereby further promoting the occurrence and development of hypertensive cerebrovascular disease.
本研究旨在探讨血清脂联素(ADPN)、高敏 C 反应蛋白(hs-CRP)和可溶性细胞间黏附分子-1(sICAM-1)水平与高血压性脑血管并发症的关系。选取 2016 年 12 月至 2018 年 12 月甘肃省医院收治的 50 例高血压性脑血管病患者为实验组,选择同期体检的 50 例正常人作为对照组。记录血压、心率及并发症发生情况,检测血清血脂指标,采用 ELISA 法检测血清 ADPN、hs-CRP、sICAM-1 含量;神经功能指标;脑源性神经营养因子(BDNF);神经元特异性烯醇化酶(NSE)。实验组的天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、血尿素氮(BUN)和血清肌酐(SCR)含量明显高于对照组( < 0.05);脑血管并发症发生率、收缩压、舒张压和心率均升高( < 0.05);总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、hs-CRP 和 sICAM-1 含量明显升高( < 0.05);而 ADPN 和 HDL 含量明显降低( < 0.05)。此外,实验组收缩期血流速度(Vs)、舒张期血流速度(Vd)和平均血流速度(Vm)明显降低,搏动指数(PI)明显升高( < 0.05)。实验组 S100 和 NSE 水平明显升高,BDNF 水平明显降低( < 0.05)。在高血压性脑血管病患者中,ADPN 水平下降;hs-CRP 和 sICAM-1 水平升高;脑血管并发症发生率升高;血脂、脑血管血流动力学和神经功能指标发生变化,从而进一步促进高血压性脑血管病的发生和发展。
