Wu Wei Zi is the dried fruit of (Turcz.) Baill. or Rehd. et Wils. (family Magnoliaceae). As a homology of medicine and food, it has been widely used in China for thousands of years, to tonify the kidney, and ameliorate neurological, cardiovascular, liver, and gastrointestinal disorders. As its increasing health benefits and pharmacological value, many literatures have reported that the combination of Wu Wei Zi in patients has led to fluctuations in the blood level of the combined drug. Therefore, it is extremely important to evaluate its safety concern such as drug-drug interactions (DDIs) when patients are under the poly-therapeutic conditions. This review summarized the effects of Wu Wei Zi extract and its major lignan components on cytochrome P450 and P-glycoprotein activities, the change of which could induce metabolic DDIs. Our review also elaborated on the differences of the major lignan components of the two species, as well as the absorption, distribution, metabolism, and elimination of the major lignans. In conclusion, these results would enhance our understanding of the DDI mechanisms involving Wu Wei Zi, and may potentially untangle some differing and conflicting results in the future.
五味子为木兰科植物五味子(Schisandra chinensis (Turcz.) Baill.)或华中五味子(Schisandra sphenanthera Rehd. et Wils.)的干燥成熟果实。作为药食同源的物质,它在中国已被广泛应用了数千年,用于补肾以及改善神经、心血管、肝脏和胃肠道疾病。随着其健康益处和药理价值的不断增加,许多文献报道五味子与患者体内联合用药会导致联合用药血液水平的波动。因此,在患者接受多药治疗的情况下,评估其诸如药物相互作用(DDIs)等安全性问题极为重要。本综述总结了五味子提取物及其主要木脂素成分对细胞色素P450和P-糖蛋白活性的影响,其变化可能会引发代谢性药物相互作用。我们的综述还阐述了这两种五味子主要木脂素成分的差异,以及主要木脂素的吸收、分布、代谢和排泄情况。总之,这些结果将增进我们对涉及五味子的药物相互作用机制的理解,并可能在未来解决一些不同且相互矛盾的结果。
