The prevalence of activities of daily living (ADL) in patients with heart failure (HF) reported in current studies were inconsistent, ranging from 11.1 to 70.5%. The purpose of this study is to quantify the prevalence of ADL impairment in HF patients.
PubMed, Embase, Cochrane, CINAHL, CNKI, SinoMed, VIP, and Wanfang databases were systematically searched for relevant studies (up to March 2, 2022). Cross-sectional, case-control, or cohort studies with detailed descriptions of overall ADL impairment in HF were included. Stata 16.0 was used for statistical analysis. Fixed-effect or random-effect model was adopted according to heterogeneity which was evaluated by Cochran's and values. Sensitivity analysis, subgroup analysis, and meta-regression were performed to investigate the sources of heterogeneity.
A total of 12 studies with 15,795 HF patients were included in the meta-analysis, and the pooled prevalence of ADL impairment in patients with HF was 38.8% (95%CI: 28.2-49.3%; = 99.5%, < 0.001). No possible sources of heterogeneity were found in subgroup analysis and meta-regression. Funnel plots and Egger's test showed no publication bias ( = 0.595).
The prevalence of ADL impairment is relatively high in HF patients. Differences in the prevalence of ADL impairment in patients with HF may be influenced by country, region, and assessment time. We suggest that more researchers could focus on the changes of ADL impairment in HF patients during different disease periods in different regions and countries.
目前研究报道的心力衰竭(HF)患者日常生活活动能力(ADL)的患病率不一致,范围为11.1%至70.5%。本研究的目的是量化HF患者ADL受损的患病率。
系统检索PubMed、Embase、Cochrane、CINAHL、中国知网、中国生物医学文献数据库、维普和万方数据库中相关研究(截至2022年3月2日)。纳入对HF患者总体ADL受损情况有详细描述的横断面、病例对照或队列研究。采用Stata 16.0进行统计分析。根据异质性采用固定效应或随机效应模型,异质性通过Cochran's Q值和I²值进行评估。进行敏感性分析、亚组分析和Meta回归以探究异质性来源。
Meta分析共纳入12项研究,涉及15795例HF患者,HF患者ADL受损的合并患病率为38.8%(95%CI:28.2 - 49.3%;I² = 99.5%,P < 0.001)。亚组分析和Meta回归未发现可能的异质性来源。漏斗图和Egger检验显示无发表偏倚(P = 0.595)。
HF患者ADL受损的患病率相对较高。HF患者ADL受损患病率的差异可能受国家、地区和评估时间的影响。我们建议更多研究人员关注不同地区和国家HF患者在不同疾病阶段ADL受损情况的变化。
