Abstract

BACKGROUND

To address the pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), vaccination efforts were initiated across the globe in December 2020 and are continuing. We report the onset interval and clinical presentations of ocular adverse effects following SARS-CoV-2 vaccination.

METHODS

For this narrative review, articles in the English language, published between 1 January 2020 to 1 September 2022, were included to formulate a list of the reported ocular adverse effects of different COVID-19 vaccines.

RESULTS

During this period, ocular adverse effects have been reported with BNT162b2 (Pfizer), mRNA-1273 (Moderna), AZD-1222 (AstraZeneca), and Ad26.COV2.S (Johnson & Johnson) vaccines. Endothelial graft rejection, herpes simplex virus keratitis, herpes zoster ophthalmicus, anterior uveitis, eyelid edema, purpuric rashes, ischemic optic neuropathy, and cranial nerve palsies were the most reported with BNT163b2. Retinal hemorrhages, vascular occlusions, and angle closure glaucoma were the most reported with AZD-1222. Most of the ocular adverse effects reported in the literature had a good to fair prognosis with appropriate management.

CONCLUSIONS

Evidence regarding the ocular adverse effects does not outweigh the benefits of SARS-CoV-2 vaccination in patients with pre-existing systemic or ophthalmic diseases. This review provides insights into the possible temporal association between reported ocular adverse events and SARS-CoV-2 vaccines; however, further investigations are required to identify the link between potential causality and pathological mechanisms.

为应对严重急性呼吸综合征冠状病毒2(SARS-CoV-2)引发的大流行,2020年12月全球启动了疫苗接种工作,且仍在继续。我们报告了SARS-CoV-2疫苗接种后眼部不良反应的发病间隔和临床表现。

方法

对于本叙述性综述,纳入了2020年1月1日至2022年9月1日期间发表的英文文章,以列出不同新冠疫苗报告的眼部不良反应清单。

结果

在此期间,已报告BNT162b2(辉瑞)、mRNA-1273(莫德纳)、AZD-1222(阿斯利康)和Ad26.COV2.S(强生)疫苗存在眼部不良反应。内皮移植排斥、单纯疱疹病毒性角膜炎、带状疱疹性眼病、前葡萄膜炎、眼睑水肿、紫癜性皮疹、缺血性视神经病变和颅神经麻痹是BNT163b2报告最多的不良反应。视网膜出血、血管阻塞和闭角型青光眼是AZD-1222报告最多的不良反应。文献中报告的大多数眼部不良反应经适当处理后预后良好至中等。

结论

对于患有全身性或眼科疾病的患者,关于眼部不良反应的证据并不超过SARS-CoV-2疫苗接种的益处。本综述深入探讨了报告的眼部不良事件与SARS-CoV-2疫苗之间可能的时间关联;然而,需要进一步研究以确定潜在因果关系与病理机制之间的联系。
