Abstract

BACKGROUND

Pain from subacromial pain syndrome (SAPS) can limit individuals' daily activities and reduce physical performance. The effectiveness of graded motor imagery (GMI) training for this condition remains unexplored. This prospective, randomized controlled trial aimed to determine the effects of GMI training in patients with chronic painful SAPS.

METHODS

Forty-two patients with SAPS were randomly assigned to the GMI (n = 21) and the control (n = 21) groups. Primary measures were the visual analogue scale (VAS), and the Disability of the Arm, Shoulder, and Hand (DASH) questionnaire. Secondary measures were Active Range of Motion (AROM), muscle strength, lateralization performance and Kinesthetic and Visual Imagery Questionnaire (KVIQ). Evaluations occurred at baseline, postintervention (6 weeks), and after a 6-week follow up.

RESULTS

The GMI group showed significantly greater improvements in both primary and secondary outcomes compared to the control group (p < 0.001). Significantly larger effect sizes were found in favor of the GMI group for VAS, DASH, abduction, external rotation AROM, muscle strength, lateralization performance, and KVIQ (ηp < 0.14, p < 0.05, for all).

CONCLUSIONS

The integration of GMI training into conventional physiotherapy for SAPS rehabilitation provides more effective clinical results in improving pain intensity and increasing functionality.

背景

肩峰下疼痛综合征(SAPS)引起的疼痛会限制个体的日常活动并降低身体机能。分级运动想象(GMI)训练对这种情况的有效性尚未得到探索。这项前瞻性随机对照试验旨在确定GMI训练对慢性疼痛性SAPS患者的影响。

方法

42例SAPS患者被随机分为GMI组(n = 21)和对照组(n = 21)。主要测量指标为视觉模拟量表(VAS)和手臂、肩部和手部功能障碍(DASH)问卷。次要测量指标为主动活动范围(AROM)、肌肉力量、定位表现以及动觉和视觉想象问卷(KVIQ)。在基线、干预后(6周)和6周随访后进行评估。

结果

与对照组相比,GMI组在主要和次要结局方面均有显著更大的改善(p < 0.001)。在VAS、DASH、外展、外旋AROM、肌肉力量、定位表现和KVIQ方面,发现明显更大的效应量支持GMI组(所有指标的ηp < 0.14,p < 0.05)。

结论

将GMI训练纳入SAPS康复的传统物理治疗中,在改善疼痛强度和增加功能方面提供了更有效的临床效果。
