伊朗东北部孕妇中症状性新冠病毒感染的患病率及其与妊娠结局的关系:一项病历回顾性研究

Abstract

INTRODUCTION

The impact of COVID-19 on pregnancy outcomes is not well known. Reviewing clinical records is a useful way to answer clinical questions. The aim of the present study was to determine the prevalence of symptomatic COVID-19 Infection among pregnant women and its relationship with pregnancy outcomes in North East Iran by reviewing medical records.

MATERIALS & METHODS: The present study is a retrospective review of the medical records of Iranian pregnant women covered by the comprehensive health service centers of Mashhad, Iran, using the consensus-based sampling. The data was related to 100,000 pregnant women, who was monitored in the screening program of pregnant mothers during the period of COVID-19 pandemic from March 2020 to November 2021, and recorded in the electronic health record dataset of Mashhad University of Medical Sciences called as SINA system. A self-structured and valid checklist containing demographic characteristics, medical records, nutritional patterns, data on pregnancy-neonatal outcomes and data related to COVID-19 was used to collect data. Data analysis was carried out by IBM SPSS software version 25 using Chi-square and Independent t-test.

FINDINGS

Out of 100,000 pregnant women, 2,345 (2.345%) were infected with COVID-19. There was a significant difference between healthy and infected pregnant women in terms of age, underlying diseases, diabetes, type of delivery, and birth weight (P < 0.001). Infected pregnant women were older, had underlying diseases and diabetes, underwent cesarean section and their new born had lower birth weight. There were no significant differences in chronic hypertension, anemia, lifestyle and other pregnancy outcomes including gestational age at delivery and maternal death between the two groups.

CONCLUSION

COVID-19 during pregnancy may be a disease with significant consequences for the pregnancy outcome. Providing comprehensive care and vaccination for pregnant women is important and reduces the possibility of adverse consequences for the mother and her baby.

2019冠状病毒病(COVID-19)对妊娠结局的影响尚不清楚。回顾临床记录是回答临床问题的一种有用方法。本研究的目的是通过回顾病历,确定伊朗东北部孕妇中症状性COVID-19感染的患病率及其与妊娠结局的关系。

材料与方法

本研究是一项回顾性研究,采用基于共识的抽样方法,对伊朗马什哈德综合健康服务中心覆盖的伊朗孕妇的病历进行回顾。数据来自100,000名孕妇,她们在2020年3月至2021年11月COVID-19大流行期间的孕妇筛查项目中接受监测,并记录在马什哈德医科大学的电子健康记录数据集(称为SINA系统)中。使用一份包含人口统计学特征、病历、营养模式、妊娠-新生儿结局数据以及与COVID-19相关数据的自拟有效检查表来收集数据。数据分析使用IBM SPSS软件25版进行卡方检验和独立t检验。

研究结果

在100,000名孕妇中,2345名(2.345%)感染了COVID-19。健康孕妇和感染孕妇在年龄、基础疾病、糖尿病、分娩方式和出生体重方面存在显著差异(P < 0.001)。感染的孕妇年龄较大,有基础疾病和糖尿病,接受剖宫产,其新生儿出生体重较低。两组在慢性高血压、贫血、生活方式以及其他妊娠结局(包括分娩时的孕周和孕产妇死亡)方面没有显著差异。

结论

孕期感染COVID-19可能是一种对妊娠结局有重大影响的疾病。为孕妇提供全面护理和疫苗接种很重要,可降低对母亲及其婴儿产生不良后果的可能性。
