In order to analyze the different features of the social network, social support system and health status of elderly people, a survey was performed of the social environment and health related issues among 70 year-old men and women in Sapporo, a large city, and Yubari, a small city which has experienced coal mine closures, both in Hokkaido prefecture. The results were as follows: 1. In Yubari, the health status of the elderly living alone was better than those with other familial categories such as living with their spouse, or with the families of their children. In contrast, the status of the elderly living alone in Sapporo was worse than the others. The mean numbers of out-patient visits and hospital admissions during the previous one year were higher among elderly people living alone in both areas. 2. There were large differences in social support and social network in the two areas and among the living status of the elderly. In particular, persons living alone in Sapporo had statistically significantly lower amount of support than others. Elderly people in Sapporo had fewer contacts with their neighbors and only a small number of them participated in community-based social associations compared with the same age groups in the city of Yubari. 3. An extremely small number of the elderly were provided formal support from official personnel such as public health nurses, district welfare commissioners and so on. The results of this study suggest the need for more formal and informal networks to provide for social support for those elderly men and women living alone in large cities.
为分析老年人社交网络、社会支持系统及健康状况的不同特征,对北海道地区大城市札幌和经历煤矿关闭的小城市夕张市70岁男女的社会环境及健康相关问题进行了调查。结果如下:1. 在夕张市,独居老年人的健康状况优于与配偶或子女家庭共同生活等其他家庭类型的老年人。相反,札幌市独居老年人的健康状况比其他人差。两个地区独居老年人前一年的门诊就诊次数和住院次数均值都更高。2. 两个地区以及老年人生活状况在社会支持和社交网络方面存在很大差异。特别是,札幌市独居者获得的支持在统计上显著低于其他人。与夕张市同年龄段人群相比,札幌市老年人与邻居的联系较少,只有少数人参与社区社会活动。3. 极少数老年人得到公共卫生护士、地区福利专员等官方人员的正式支持。本研究结果表明,需要更多正式和非正式网络为大城市中独居的老年男女提供社会支持。
