Abstract

OBJECTIVE

To study the role of the polymorphonuclear neutrophil (PMN) apoptosis and expression of Fas and caspase-3 in the systemic inflammatory response syndrome (SIRS).

METHODS

Eight acute pancreatitis patients with SIRS, 6 healthy control subjects were enrolled to study apoptosis of PMN in peripheral blood samples, expression of Fas/Fas ligand (FasL) and caspase-3 in the PMN, the levels of serum interleukin-6 (IL-6) and IL-8 were observed.

RESULTS

Spontaneous apoptosis was significantly delayed in PMN from the SIRS patients with higher serum IL-6 and IL-8 levels compared with controls (both P<0.01). PMN apoptosis rate in peripheral blood of patients with SIRS was lowered than that of controls (P<0.01). The expressions of Fas and caspase-3 in the peripheral circulating PMN were higher in the controls than those in the SIRS patients (both P<0.01). Serum FasL expression was not found by Western blotting 24 hours after culture of PMN in vitro.

CONCLUSION

Peripheral circulating PMN from acute pancreatitis patients with SIRS show delayed apoptosis, decreased expressions of Fas and caspase-3, and prolonged PMN survival may contribute to the development of systemic inflammatory injury characteristic of SIRS.

研究多形核中性粒细胞(PMN)凋亡及Fas和半胱天冬酶-3(caspase-3)表达在全身炎症反应综合征(SIRS)中的作用。

方法

纳入8例伴有SIRS的急性胰腺炎患者及6例健康对照者,研究外周血样本中PMN的凋亡情况、PMN中Fas/Fas配体(FasL)和caspase-3的表达,观察血清白细胞介素-6(IL-6)和IL-8水平。

结果

与对照组相比,伴有SIRS的急性胰腺炎患者血清IL-6和IL-8水平较高,其PMN的自发凋亡明显延迟(均P<0.01)。SIRS患者外周血PMN凋亡率低于对照组(P<0.01)。对照组外周循环PMN中Fas和caspase-3的表达高于SIRS患者(均P<0.01)。体外培养PMN 24小时后,Western印迹法未检测到血清FasL表达。

结论

伴有SIRS的急性胰腺炎患者外周循环PMN显示凋亡延迟、Fas和caspase-3表达降低,PMN存活时间延长可能有助于SIRS特征性全身炎症损伤的发展。
