The purpose of this study was to review the effect of low-level laser therapy (LLLT) in the management of breast cancer-related lymphedema (BCRL).
A systematic review of seven databases for clinical trials for LLLT in the management of BCRL published between 1990 and 2011 was performed.
A total of eight studies on 230 patients were found. The methodological qualities of the selected studies were assessed with the Physiotherapy Evidence Database scale, and the studies were categorized according to Sackett's levels of evidence. Five studies were graded at evidence level II. Two studies were graded at evidence level III, and the remaining study was graded at evidence level V.
There is moderate to strong evidence for the effectiveness of LLLT for the management of BCRL from five small studies of acceptable methodological quality. A dose of 1-2 J/cm(2) per point applied to several points covering the fibrotic area can reduce limb volume following BCRL. Further well-designed, large-scale studies are required to determine more precisely how effective LLLT may be in BCRL.
本研究旨在回顾低水平激光疗法(LLLT)在乳腺癌相关淋巴水肿(BCRL)管理中的作用。
对 1990 年至 2011 年间发表的关于 LLLT 治疗 BCRL 的临床试验进行了七个数据库的系统回顾。
共发现了 8 项涉及 230 例患者的研究。采用物理治疗证据数据库量表评估所选研究的方法学质量,并根据 Sackett 的证据水平对研究进行分类。5 项研究的证据水平为 II 级,2 项研究的证据水平为 III 级,其余 1 项研究的证据水平为 V 级。
从 5 项具有可接受方法学质量的小型研究中得出了中等至强证据,表明 LLLT 对 BCRL 的管理有效。在纤维化区域的多个点上应用 1-2 J/cm²/点的剂量可减少 BCRL 后肢体体积。需要进一步设计良好的大规模研究来更准确地确定 LLLT 在 BCRL 中的有效性。
