Abstract

PURPOSE

To report a case of Vogt-Koyanagi-Harada (VKH) disease and describe the imaging findings by means of optical coherence tomography angiography (OCTA).

METHODS

Medical and ophthalmological history, ophthalmological examination, laboratory evaluation, B-scan ultrasonography, fluorescein and indocyanine angiography, and optical coherence tomography (OCT) were performed at baseline, as well as OCTA.

RESULTS

A 50-year-old healthy female presented with decreased vision in both eyes. A Topcon DRI OCT Triton Plus swept source OCT system was used to visualize and evaluate the retinal and choroidal vascular plexus. Patchy and confluent dark areas in the superficial and deep retinal capillary plexus and choriocapillaris corresponded to areas of hypoperfusion, analyzed as areas of ischemia.

CONCLUSIONS AND IMPORTANCE

VKH disease is characterized by ocular, neurological, and integumentary findings in its complete form. We present a case of incomplete disease in a 50-year-old female evaluated by means of OCTA which is a novel technique that provides depth-resolved images of the retina and choroidal microvasculature without dye injection that allows better visualization and detailed evaluation of the retinal and choroidal vascular plexus.

方法

在基线时进行了医学和眼科病史、眼科检查、实验室评估、B超检查、荧光素和吲哚菁绿血管造影以及光学相干断层扫描(OCT),以及OCTA检查。

结果

一名50岁健康女性出现双眼视力下降。使用拓普康DRI OCT Triton Plus扫频源OCT系统观察和评估视网膜和脉络膜血管丛。视网膜浅、深层毛细血管丛和脉络膜毛细血管层中散在和融合的暗区对应于灌注不足区域,分析为缺血区域。

结论与意义

VKH病的典型表现为眼部、神经和皮肤方面的症状。我们报告了1例50岁女性的不完全型VKH病病例,通过OCTA进行评估,OCTA是一种新技术,无需注射染料即可提供视网膜和脉络膜微血管系统的深度分辨图像,从而能更好地观察和详细评估视网膜和脉络膜血管丛。
