Abstract

OBJECTIVES: Mind-body exercise has been generally recognized as a beneficial strategy to improve mental health in those with Chronic Obstructive Pulmonary Disease (COPD). However, to date, no attempt has been made to collate this literature. The aim of the present study was to systematically analyze the effects of mind-body exercise for COPD patients with anxiety and depression and provide scientific evidence-based exercise prescription. METHODS: both Chinese and English databases (PubMed, the Cochrane Library, EMBASE, Web of Science, Google Scholar, Chinese National Knowledge Infrastructure, Wanfang, Baidu Scholar) were used as sources of data to search randomized controlled trials (RCT) relating to mind-body exercise in COPD patients with anxiety and depression that were published between January 1982 to June 2019. 13 eligible RCT studies were finally used for meta-analysis. RESULTS: Mind-body exercise (tai chi, health qigong, yoga) had significant benefits on COPD patients with anxiety (SMD= -0.76, 95% CI -0.91 to -0.60, =0.04, I=47.4%) and depression (SMD= -0.86, 95% CI -1.14 to -0.58, =0.000, I=71.4%). Sub-group analysis indicated that, for anxiety, 30-60 min exercise session for 24 weeks of health qigong or yoga had a significant effect on patients with COPD who are more than 70 years and have more than a 10-year disease course. For depression, 2-3 times a week, 30-60 min each time of health qigong had a significant effect on patients with COPD patients who are more than 70 years old and have less than a 10-year disease course. CONCLUSIONS: Mind-body exercise could reduce levels of anxiety and depression in those with COPD. More robust RCT are required on this topic.

目的:身心锻炼已被普遍认为是改善慢性阻塞性肺疾病(COPD)患者心理健康的有益策略。然而,迄今为止,尚未尝试对此类文献进行整理。本研究旨在系统分析身心锻炼对 COPD 合并焦虑和抑郁患者的影响,并提供基于科学证据的运动处方。

方法:检索中国和英文数据库(PubMed、Cochrane 图书馆、EMBASE、Web of Science、Google Scholar、中国知网、万方、百度学术),搜集 1982 年 1 月至 2019 年 6 月发表的有关 COPD 合并焦虑和抑郁患者进行身心锻炼的随机对照试验(RCT)。最终纳入 13 项符合条件的 RCT 进行荟萃分析。

结果:身心锻炼(太极拳、气功、瑜伽)对 COPD 合并焦虑(SMD=-0.76,95%CI-0.91 至-0.60,=0.04,I²=47.4%)和抑郁(SMD=-0.86,95%CI-1.14 至-0.58,=0.000,I²=71.4%)患者均有显著获益。亚组分析表明,气功或瑜伽 30-60 分钟、24 周疗程,对 70 岁以上、病程 10 年以上的 COPD 患者焦虑有显著改善作用;气功每周 2-3 次、每次 30-60 分钟,对 70 岁以上、病程 10 年以下的 COPD 患者抑郁有显著改善作用。

结论:身心锻炼可以降低 COPD 患者的焦虑和抑郁水平。还需要更多关于该主题的高质量 RCT 研究。
