Abstract

BACKGROUND

Lymph node metastasis (LNM) is associated with increased risk of recurrence and poor prognosis in papillary thyroid cancer (PTC). Novel non-invasive biomarkers for the prediction of LNM in PTC patients are still urgently needed. In this study, the relationship between the expression of plasma exosomal microRNAs (miRNAs) and LNM was analyzed. Further, we aimed to explore if exosomal miRNAs can serve as indicators of LNM in PTC patients.

METHODS

A total of 64 PTC patients who underwent total thyroidectomy and neck dissection from June 2018 to July 2018 in West China Hospital were enrolled in this study. Plasma exosomes were isolated by exoRNeasy Serum/Plasma Maxi Kit. The levels of selected exosomal miRNAs were detected by real-time quantitative PCR (qRT-PCR). Cox proportional hazard analyses and receiver operating characteristic (ROC) curves were conducted to evaluate the predictive efficiency. Furthermore, PTC cell lines with transfection of miRNA mimics/inhibitors were used to verify the functions of exosomal miRNAs.

RESULTS

49 PTC patients with LNM and 15 without LNM were included in the present study. Exosomal miR-146b-5p and miR-222-3p were both significantly upregulated in patients with LNM (P values were 0.008 and 0.015, respectively). ROC analyses revealed that the areas under the curves (AUCs) of miR-146b-5p and miR-222-3p for LNM prediction were 0.811 and 0.834, respectively. Moreover, the AUC increased to 0.895 when the two miRNAs used together. Wound healing assays and transwell assays showed that miR-146b-5p and miR-222-3p significantly enhanced the migration and invasion ability of PTC cells in vitro.

CONCLUSION

Plasma exosomal miR-146b-5p and miR-222-3p could serve as potential biomarkers for LNM in PTC.

背景

淋巴结转移(LNM)与甲状腺乳头状癌(PTC)的复发风险增加及预后不良相关。目前仍迫切需要用于预测PTC患者LNM的新型非侵入性生物标志物。在本研究中,分析了血浆外泌体微小RNA(miRNA)表达与LNM之间的关系。此外,我们旨在探讨外泌体miRNA是否可作为PTC患者LNM的指标。

方法

本研究纳入了2018年6月至2018年7月在华西医院接受全甲状腺切除术和颈部淋巴结清扫术的64例PTC患者。采用exoRNeasy血清/血浆Maxi试剂盒分离血浆外泌体。通过实时定量PCR(qRT-PCR)检测所选外泌体miRNA的水平。进行Cox比例风险分析和受试者工作特征(ROC)曲线分析以评估预测效率。此外,使用转染了miRNA模拟物/抑制剂的PTC细胞系来验证外泌体miRNA的功能。

结果

本研究纳入了49例有LNM的PTC患者和15例无LNM的患者。外泌体miR-146b-5p和miR-222-3p在有LNM的患者中均显著上调(P值分别为0.008和0.015)。ROC分析显示,miR-146b-5p和miR-222-3p预测LNM的曲线下面积(AUC)分别为0.811和0.834。此外,当两种miRNA联合使用时,AUC增加至0.895。伤口愈合试验和Transwell试验表明,miR-146b-5p和miR-222-3p显著增强了PTC细胞在体外的迁移和侵袭能力。

结论

血浆外泌体miR-146b-5p和miR-222-3p可作为PTC患者LNM的潜在生物标志物。
