BACKGROUND/OBJECTIVES: To investigate the agreement between optical coherence tomography angiography (OCT-A) and non-mydriatic retinal camera (NMRC) in estimating the optic nerve head (ONH) parameters and evaluate the associations between peripapillary vessel density (VD) with ONH parameters assessed by both devices.
The OCT-A and NMRC images of 262 participants (138 eyes were diagnosed with POAG, 63 were glaucoma suspect, and 61 were healthy subjects) were screened retrospectively. The vertical cup-to-disc ratio (VCDR), cup-to-disc area ratio (CDAR), rim area (RA), disc area (DA), and cup volume (CV) were recorded using both devices. Peripapillary VD data were also recorded using OCT-A. The intraclass correlation coefficient (ICC) and Bland-Altman plots were obtained to investigate the agreement between the devices. Age- and intraocular pressure (IOP)-corrected associations between the VD and ONH parameters measured by the two devices were analyzed using linear regression models.
ICC revealed excellent agreement for VCDR and CDAR, but poor agreement for RA, DA, and CV (ICC = 0.801, 0.793, 0.445, 0.298, and 0.527, respectively). The Bland-Altman plots showed poor agreement for VCDR and CDAR with a large span of limits of agreement (0.500 and 0.528, respectively) and significant proportional biases (P < 0.05). VD was found to be strongly associated with the VCDR and CDAR measurements of both devices (P < 0.001), but the associations were stronger for the measurements of NMRC.
The disagreement between the devices should be considered in clinical practice. The associations between the VD and ONH parameters proved once again the importance of the ONH parameters in terms of the management of glaucoma patients.
背景/目的:研究光学相干断层扫描血管造影(OCT-A)与免散瞳视网膜相机(NMRC)在评估视神经乳头(ONH)参数方面的一致性,并评估两种设备所测视乳头周围血管密度(VD)与ONH参数之间的相关性。
回顾性筛选262名参与者的OCT-A和NMRC图像(138只眼被诊断为原发性开角型青光眼,63只为青光眼可疑患者,61只为健康受试者)。使用两种设备记录垂直杯盘比(VCDR)、杯盘面积比(CDAR)、视盘边缘面积(RA)、视盘面积(DA)和杯体积(CV)。还使用OCT-A记录视乳头周围VD数据。获得组内相关系数(ICC)和Bland-Altman图以研究两种设备之间的一致性。使用线性回归模型分析两种设备测量的VD与ONH参数之间经年龄和眼压(IOP)校正后的相关性。
ICC显示VCDR和CDAR具有良好的一致性,但RA、DA和CV的一致性较差(ICC分别为0.801、0.793、0.445、0.298和0.527)。Bland-Altman图显示VCDR和CDAR的一致性较差,一致性界限跨度较大(分别为0.50 和0.528)且存在显著的比例偏差(P<0.05)。发现VD与两种设备测量的VCDR和CDAR均密切相关(P<0.001),但NMRC测量的相关性更强。
在临床实践中应考虑两种设备之间的差异。VD与ONH参数之间的相关性再次证明了ONH参数在青光眼患者管理中的重要性。
