Abstract

OBJECTIVE

To investigate the correlation between the platelet-to-lymphocyte ratio (PLR) and diabetic foot ulcer (DFU) in patients with type 2 diabetes mellitus (T2DM).

METHOD

From January 2018 to August 2019, 206 patients with T2DM admitted to the Central Hospital of Wuhan, China, were enrolled in this study, including 104 patients with DFU (DFU group) and 102 patients without DFU (T2DM group). During the same period, 90 healthy subjects were randomly screened as normal controls (NC group). The correlation between PLR and DFU in patients with T2DM was explored by comparing the PLR of the subjects in the three groups.

RESULTS

The PLRs of the DFU and T2DM groups were higher than that of the NC group, whereas the PLR of the DFU group was higher than that of the T2DM group (p < 0.05). PLR was positively correlated with the Wagner DFU grade (p < 0.001). Based on logistic regression analysis, PLR was found to be an independent risk factor for DFU (OR =1.029, 95% CI: 1.019 ~ 1.039, p < 0.001). The receiver operating characteristic curve analysis of the PLR showed that the area under the curve of the PLR for predicting diabetic foot ulcer was 0.776 (p < 0.001), and the analysis determined that the optimal critical value of the PLR for predicting DFU was 147.6.

CONCLUSION

The PLR is significantly elevated in patients with DFU and positively correlated with the Wagner DFU grade, which might be a valuable marker for early diagnosis and assessment of severity of DFU.

探讨血小板与淋巴细胞比值(PLR)与 2 型糖尿病(T2DM)患者糖尿病足溃疡(DFU)的相关性。

方法

本研究纳入 2018 年 1 月至 2019 年 8 月期间在中国武汉中心医院就诊的 206 例 T2DM 患者,包括 104 例 DFU 患者(DFU 组)和 102 例无 DFU 患者(T2DM 组)。同期随机筛选 90 例健康受试者作为正常对照组(NC 组)。通过比较三组受试者的 PLR,探讨 PLR 与 T2DM 患者 DFU 的相关性。

结果

DFU 组和 T2DM 组的 PLR 均高于 NC 组,而 DFU 组的 PLR 高于 T2DM 组(p<0.05)。PLR 与 Wagner DFU 分级呈正相关(p<0.001)。基于 logistic 回归分析,PLR 是 DFU 的独立危险因素(OR=1.029,95%CI:1.019~1.039,p<0.001)。PLR 预测糖尿病足溃疡的受试者工作特征曲线分析显示,PLR 预测糖尿病足溃疡的曲线下面积为 0.776(p<0.001),分析确定 PLR 预测 DFU 的最佳临界值为 147.6。
