The aging population has become a severe public health issue both in China and globally. Over 180 million elderly people in China have chronic diseases, with 75% of them suffering from 2 or more chronic diseases, posing significant threats to their independence and quality of life, and imposing immense mental and disease burdens on patients and their families. So our research analyzes the influencing factors based on the current acceptance status of advance care planning (ACP) among elderly patients with chronic diseases. Using convenience sampling, 120 elderly inpatients from the geriatrics department of a tertiary hospital in Beijing, from June 2023 to March 2024, were selected. Surveys included a general information questionnaire, an ACP acceptance questionnaire, and the revised version of the Terminal Care Experience and Death Attitude Profile. The total ACP acceptance score among elderly patients with chronic diseases was 73.98 ± 9.84 points. Factors influencing ACP acceptance included fear of death, whether the patient had cared for a dying relative, duration of chronic illness, presence of complications, natural acceptance, and religious beliefs (P < .05). The acceptance level of ACP among elderly patients with chronic diseases is moderately high. Medical staff should provide more choices to enhance the life value of elderly patients with chronic diseases based on fully respecting their wishes.
老龄化人口已成为中国乃至全球严峻的公共卫生问题。中国有超过1.8亿老年人患有慢性病,其中75%的人患有一种以上慢性病,这对他们的独立性和生活质量构成重大威胁,并给患者及其家庭带来巨大的精神和疾病负担。因此,我们的研究基于老年慢性病患者对预先护理计划(ACP)的当前接受状况分析影响因素。采用便利抽样法,选取了2023年6月至2024年3月期间北京某三级医院老年科的120名老年住院患者。调查包括一般信息问卷、ACP接受情况问卷以及临终关怀体验与死亡态度量表修订版。老年慢性病患者的ACP接受总分为73.98 ± 9.84分。影响ACP接受度的因素包括对死亡的恐惧、患者是否照顾过临终亲属、慢性病病程、是否存在并发症、自然接受度以及宗教信仰(P < 0.05)。老年慢性病患者对ACP的接受程度中等偏高。医护人员应在充分尊重老年慢性病患者意愿的基础上,提供更多选择以提升他们的生命价值。
