BACKGROUND/AIM: The parotid glands in the head and neck are organs at risks (OARs) adjacent to high dose region and dose of OARs might be increased during the course of radiotherapy. The influence factors of the dose distribution for the parotid glands were investigated in terms of weight loss and mandibular rotation for head and neck cancers treated with volumetric modulated arc therapy (VMAT).
Ten oropharyngeal cancer patients (OPC) who underwent VMAT were enrolled. The dose volume histogram (DVH) parameters of the parotid glands and planning target volume (PTV) were compared between the planning computed tomography (CT) and the on board imager (OBI) at 1, 5, 10, 15, and 20 fractions.
The variation of dose distribution in PTV was not observed in both factors. The relationship between the mandibular rotation and dose difference for the right and left parotid glands (linear regression, r=0.1577 and -0.689) showed a slighty stronger correlation with dose difference than the weight loss (linear regression, r=-0.079 and -0.547).
The mandibular rotation tends to have a large influence on dose distribution of the parotid glands for head and neck cancers treated with VMAT.
背景/目的:头部和颈部的腮腺是紧邻高剂量区域的危险器官(OAR),在放射治疗过程中,OAR 的剂量可能会增加。本研究旨在探讨调强弧形治疗(VMAT)治疗头颈部癌症时,体重减轻和下颌旋转对腮腺剂量分布的影响因素。
纳入 10 例接受 VMAT 治疗的口咽癌(OPC)患者。比较了计划 CT(planning computed tomography,PTV)和在线成像(on board imager,OBI)上第 1、5、10、15 和 20 次分割时腮腺和计划靶区(planning target volume,PTV)的剂量体积直方图(dose volume histogram,DVH)参数。
在这两个因素中,PTV 内的剂量分布变化不明显。左右腮腺剂量差异与下颌旋转的关系(线性回归,r=0.1577 和 -0.689)比体重减轻的关系(线性回归,r=-0.079 和 -0.547)稍强。
对于 VMAT 治疗的头颈部癌症,下颌旋转对腮腺剂量分布的影响较大。
