We report a case of pure choriocarcinoma of the left testis in a 27-year-old male. He consulted a physician with left back pain in August, 1990. Intravenous pyelography (IVP) revealed left hydronephrosis. Abnormal computerized tomography (CT) revealed para-aortic lymph node swelling. The chest X-ray revealed bilateral multiple coin lesions. The genital examination revealed a left testicular nodule 8 mm in diameter. Left high inguinal orchiectomy was performed. Histopathological diagnosis was pure choriocarcinoma. After 4 courses of the PVB (cisplatin, vinblastine, bleomycin) therapy, tumor regression rate of the pulmonary metastases was 41.9%. He was transferred to our hospital on December 3, 1990 and was admitted 3 days later. "COMPE" chemotherapy, consisting of cisplatin, vincristine, methotrexate, peplomycin, and etoposide, was administered. After seven courses of "COMPE" chemotherapy, he achieved a partial response (the regression rate of the pulmonary metastases and the retroperitoneal lymph node metastasis were 78.8% and 69.1%, respectively) and the residual masses in the lungs and the retroperitoneum were removed. Necrosis and xanthogranulomatous fibrosis were found in the resected material. The patient showed no evidence of disease one year after thoracotomy.
我们报告一例27岁男性左睾丸纯绒毛膜癌病例。1990年8月，他因左背痛咨询医生。静脉肾盂造影（IVP）显示左肾积水。计算机断层扫描（CT）异常显示主动脉旁淋巴结肿大。胸部X线显示双侧多发硬币状病灶。生殖器检查发现左侧睾丸有一个直径8毫米的结节。进行了左侧高位腹股沟睾丸切除术。组织病理学诊断为纯绒毛膜癌。经过4个疗程的PVB（顺铂、长春碱、博来霉素）治疗，肺转移瘤的肿瘤消退率为41.9%。1990年12月3日他被转至我院，并于3天后入院。给予由顺铂、长春新碱、甲氨蝶呤、培普利霉素和依托泊苷组成的“COMPE”化疗。经过7个疗程的“COMPE”化疗后，他获得了部分缓解（肺转移瘤和腹膜后淋巴结转移的消退率分别为78.8%和69.1%），并切除了肺和腹膜后的残留肿块。切除的组织中发现坏死和黄色肉芽肿性纤维化。开胸术后一年，患者无疾病迹象。
